<br/><br/> This study assesses the knowledge, awareness, understanding, and practices of Barangay Health Workers (BHWs) regarding Republic Act No. 10173 (Data Privacy Act of 2012) in Buhi, Camarines Sur, specifically in Poblacion barangays Sta. Elena, Sta.Clara, San Pascual, San Roque, and San Pedro. As frontline handlers of sensitive health data amid digital health transitions, BHWs (mostly female, aged 41+, high school graduates) were surveyed using a structured questionnaire with Likert scales to evaluate familiarity with privacy principles, daily practices, and implementation barriers. Key outputs reveal moderate knowledge levels: strengths in patient rights and consent but gaps in breach reporting; good practices overall with secure storage high yet digital transmission weaker; and significant challenges led by fear of errors and insufficient training, affecting those untrained despite years of service. Findings highlight urgent capacity gaps at the grassroots level, underscoring Protection Motivation Theory's role in compliance. Recommendations propose a comprehensive Barangay Data Privacy Training Plan: modular workshops by Municipal Health Office and National Privacy Commission, integration into BHW accreditation, low-cost secure templates, and annual audits with incentives. These targeted interventions equip policy makers to enhance confidentiality, integrity, and security of community health data, fostering ethical practices and patient trust in Bicol Region's primary care systems while bridging policy-practice divides.
