The quantitative type of research specifically used the descriptive methods with the questionnaire as the main data gathering tool and distributed to 60 randomly selected residents of Sagrada, Baao, Camarines Sur. The data gathered were analyzed using percentage technique, weighted mean and Fisher's exact test. Finding revealed that most of the respondents belonged to the generation Z, born between 1997 and 2012, majority were college graduates, mostly were unemployed, majority were poor whose family monthly income was below P10,957, and most travel five minutes or less to reach the Barangay health office. Consequently, human resources are frequently viewed as a challenge in the delivery of public health services; materials and supplies; equipment and facilities were often inadequate. While factors along individual behavior, physical behavior, and social determinants perceived as agree which means that both challenges and factors moderately affect the public health services delivery in Sagrada, Baao, Camarines Sur. Every plan was gradually prioritized based on the respondents' level of preference. In the entire proposed plan, first is the Health Organization which is rated as the Highest Priority, second is the Governmental Services which is rated as the High Priority, third is the Public Awareness and Schedule which is rated as Normal Priority and last is the Training which is rated as Low Priority. Though focusing on challenges and factors, the Barangay health workers and residents learned how to properly render patient-care through a holistic-based approach.
