In a highly technological society, human productivity is made more efficient through the development of electronic devices. However, the case is different in Barangay Health Centers which still uses manual storing of files and computing, evidently using traditional approach as technology in rural areas is not yet developed. With these, the researchers come up with an idea that need to build a system that will help store the health records of patients make it more organized. Generally, the purpose of this system is to make staffs lessen the manual storage of data and make it easier to manage their transaction towards keeping, updating, deleting, editing, and searching patient's information. To address the general and specific objectives of the study, the researchers used a mixed method research design and the extreme programming as the software development methodology. It is useful for collecting, analyzing, and interpreting data. The respondents of this study were the patients and barangay health center staff. The data was obtained using an interview guide, black box testing and the data was evaluated using an arithmetic mean. The research findings indicate that there are problems in the data requirements needed for the system. In addition, the developed system meets the requirements for user-acceptance, performance, and security using black box testing, which verifies and checks the system's work. As a result, the overall interpretation demonstrated that the proposed system is beneficial to the respondents and a perfect opportunity to put the barangay health center information records management system into implementation.
